Lead Tests
The Indiana Department of Health requires children receive a blood lead test at the following times:
- Between the ages of 9 and 13 months
- Between the ages of 21 and 27 months
- Any child between 28 and 72 months that does not have a record or prior blood test
Indiana statute requires that ALL children enrolled in Medicaid receive a blood lead test at
- 12 months of age
- 24 months of age
- Any child between 24 and 72 months enrolled in Medicaid, with no previous blood lead screening test
Appointments available: Wednesdays only in July.
Understanding Trust and How To Answer Any Question
To continue to build trust in vaccines, Super Shot is welcoming Todd Wolynn, MD, MMM and Chad Hermann, PhD ABD of the Trusted Messenger Program to Fort Wayne, IN, on Wednesday, August 19, 2026. They will provide training on “Understanding Trust and How to Answer Any Question”.
Vaccine counseling has become an increasingly critical component of patient care. This training will provide evidence-based communication approaches that strengthen provider patient relationships and methods to respond effectively when patients express questions, concerns, and hesitancy. This event is for any healthcare or clinical worker

- How to respond to patient questions, concerns, and hesitancy
- Methods to strengthen provider-patient trust
What to Bring to Your Appointment
Super Shot serves Medicaid, uninsured, and underinsured children from birth through age 18. All American Academy of Pediatrics (APA) recommended childhood vaccinations are available.

We offer 15+ seasonal and recommended vaccinations for uninsured and underinsured adults.

No ID is required at your appointment.
We offer a sensory-friendly exam room with a bubble wall, dimmable lights, fidget toys, and comfortable seating.

Insurance We Accept
Healthy Indiana Plan (HIP) Super Shot accepts all HIP providers at no cost to you, including: Anthem Blue Cross Blue Shield, Managed Health Services (MHS), MDWise, and CareSource.
Commercial Insurance Super Shot is a VaxCare partner, which means we can accept most commercial insurance plans. Not sure if your plan is covered? Call your insurance provider and ask:
- Are vaccines covered under my plan?
- Is VaxCare an in-network provider?
Our staff is always happy to help with insurance questions — just ask!
Never turned away. If you're uninsured or underinsured, we still want to see you. Super Shot never turns anyone away for inability to pay.
Uninsured & Underinsured
Whether you have insurance that doesn't cover vaccines or no insurance at all, Super Shot is here for you. There is a $15 administration fee per immunization — but we will never turn anyone away for inability to pay.

Childhood Vaccines at Super Shot
Super Shot provides all American Academy of Pediatrics (AAP) recommended childhood vaccines, from birth through age 18. Staying on schedule gives your child the best protection. If your child misses a dose or falls behind, don't worry. We'll pick up right where they left off.

Adult Vaccines at Super Shot
Staying current on vaccines isn't just for kids. Super Shot offers a full range of recommended adult immunizations to help keep you and your community protected.
Vaccines we offer include:
- COVID-19 — annual
- Influenza (Flu) — annual
- Respiratory Syncytial Virus (RSV) — 32–36 weeks of pregnancy; ages 50–74 with risk factors; ages 75+
- Hepatitis B (HepB) — ages 18+
- Human Papillomavirus (HPV) — ages 18–45 if you've never received it before
- Pneumococcal (Pneumonia) — ages 50+, or earlier if immunocompromised
- Shingles (Zoster) — ages 50+
- Tetanus, Diphtheria & Pertussis (Tdap) — every 10 years, or if you'll have close contact with a newborn
