What to bring

All children under the age of 18 must be accompanied by a parent or legal guardian.
Everyone must bring a current shot record to every visit.
If you have commercial insurance, please bring your insurance card.
If you or your child has sensory needs, Super Shot offers a sensory-friendly exam room with a bubble wall, dimmable lights, fidget toys, and comfortable seating. Please request this accommodation when you book your appointment or mention it to a staff member upon arrival.

Children (Under 18)

We serve Medicaid, uninsured and underinsured children from birth through 18 years of age.

All children under the age of 18 must be accompanied by a parent or legal guardian.
All CDC recommended vaccinations for children are available.

Please bring a copy of the child’s shot record.

Adults (18+)

We also serve underinsured and uninsured adults by offering a limited number of vaccinations, including Hepatitis A and B, Human Papillomavirus (HPV), Influenza, Varicella, Tetanus, Diphtheria, Pertussis (Tdap), Measles, Mumps and Rubella (MMR), Meningococcal, Pneumococcal 13valent conjugate (PCV13), Pneumococcal polysaccharide (PSV23).

Shot record and insurance card required, if available.

Paying for Immunizations

We accept all Healthy Indiana Plan (HIP) Providers – Anthem Blue Cross Blue Shield, Managed Health Services (MHS), MDWise, and CareSource. There is no fee.

Super Shot is a VaxCare partner. This means that we can accept most commercial insurance plans. Not sure if your plan covers vaccines? Call your insurance provider and to ask if vaccines are covered under your plan and if VaxCare is an in-network provider. Super Shot staff is always happy to help with insurance questions.

If you are underinsured, meaning you have insurance that does not cover immunizations, we can provide immunizations, but there is a $15 administration fee per immunization.

If you are uninsured, we can provide immunizations, but there is a $15 administration fee per immunization.

Super Shot will turn no one away for inability to pay. Please ask for more information.

Infant and Child Vaccine Schedule

By following the recommended vaccination schedule, you help protect your child against 15 vaccine-preventable diseases and the flu. If your child misses a dose or gets behind schedule, make sure they get the next dose as soon as possible.

This is a schedule of all vaccines needed from birth to 18 years of age. Additional vaccines for those six months and older include flu, COVID-19 and RSV.

Top 10 Reasons to Protect Children Through Vaccination

Parents want to do everything possible to make sure their children are healthy and protected from preventable diseases. Vaccination is the best way to do that.

Vaccination protects children from serious illness and complications of vaccine-preventable diseases which can include amputation of an arm or leg, paralysis of limbs, hearing loss, convulsions, brain damage, and death.

Organizations such as the American Academy of Pediatrics, the American Academy of Family Physicians, and the Centers for Disease Control and Prevention all strongly support protecting children with recommended vaccinations.

Vaccination protects others you care about, including family members, friends, and grandparents.

If children aren’t vaccinated, they can spread disease to other children who are too young to be vaccinated or to people with weakened immune systems, such as transplant recipients and people with cancer. This could result in long-term complications and even death for these vulnerable people.

We all have a public health commitment to our communities to protect each other and each other’s children by vaccinating our own family members.
