At Super Shot, our mission is to break down the barriers when it comes to receiving immunizations. Sometimes the biggest barrier is having immunizations available in a convenient location. Super Shot is able to offer most vaccines on site at your school or place of business.

Super Shot can bring experienced vaccinators to your location to answer questions, review shot records, and administer vaccines on-site. FAQs

What kind of space do I need to host a vaccine event?

Super Shot teams are used to giving vaccines in a variety of locations. We prefer a space that is semi-private with at least one table, a few chairs, and access to power. Larger events requiring more vaccinators will require more space. Having access to Wifi is also helpful but not always necessary.

How can participants sign up?

There are different options available that we can help you to choose from, based on the type and flow of your event. We have formats for paper, online, or by phone. We can allow for walk-in, scheduled appointments, or even a mix. Let us help you find what’s best for your community or group.

Where can participants find a copy of their shot record?

Before a routine immunization event, patients/parents can try current/former school or current/former physician. If you can’t locate one, please give us a call and we will see if we can help you.
After an event, participants can request a copy of their record by calling Super Shot*.

*Super Shot submits all vaccine information to Children and Hoosiers Immunization Registry Program (CHIRP). This registry is required for all vaccines administered in the state of Indiana. Please allow 7 business days for records to be completed.
